An employee was working to attach a safety handle to the cable portion of a pulley system designed to aid in the operation of a resistance welder. The tension on the cable was disengaged, then reengaged while the employee's left hand was on the top of the pulley system. The cable caught the tip of the employee's left little finger, causing an amputation at the first joint.

An employee was using a resistance welder when his left little fingertip was caught in the collar of the welding mechanism and amputated.

Four employees were lifting a truck bed when one employee injured their back. The injured employee sustained fractured vertebrae.

An employee was changing out a customer's windshield. The employee was carrying the windshield into the shop from outside when he slipped on ice. The windshield cut the tip of his right middle finger, resulting in a fingertip amputation.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee was repairing the suspension of a semi-trailer. He was installing an air bag to the suspension when his left ring finger was crushed between the air bag and the suspension mounting beam. The employee was hospitalized.

An employee was refueling a pressure washer when it ignited and exploded. He suffered multiple third-degree burns to his chest and hands.

A mechanic had just finished a repair on a straight truck with a flatbed , which had been lifted using a bottle jack. As the employee was about to remove the jack, it failed. The truck shifted forward, dropped down, and struck the employee's chest. He suffered broken ribs and a broken shoulder.

An employee was performing diagnostic work on a vehicle engine. As the employee was leaning over the front of the vehicle, the engine was activated. A fuel leak had produced fumes around the engine, and the ignition spark ignited them. The employee suffered second-degree burns to both hands, both biceps, and the upper chest area.

An employee was hot patching a tire, which involves lighting a flammable liquid on fire to patch the tire. The flammable liquid contacted his arm, and he sustained burns to his arms and face.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
