On or about 11am, on 4/9/2015, a pipe fitter was working off an 8 ft. A-frame ladder about 5 feet off the ground, trying to loosen a valve on the nitric acid line. The valve broke loose spraying liquid onto the employee. As the employee attempted to block the spray, he lost his balance and fell off the ladder, cracking an L3 vertebrae. He was hospitalized.

Albemarle Corporation

On November 17, 2023, a maintenance employee opened a line and residual bromine was released. The employee sustained burns to their palm and left leg and also suffered an inhalation injury.

Jody Enterprises, Inc.

An employee was picking up trash bags on a route and putting the bags in the back of a garbage truck. The employee was compacting the trash when one of the bags released vapor and dust from chlorine and pool cleaning chemicals. The vapor and dust contacted the employee's eyes and mouth and were inhaled. The employee was hospitalized with chemical irritation to the eyes and lungs.

Brainerd Chemical Midwest, LLC.

On June 2, 2023, an employee was re-installing a repaired valve. When the employee applied pressure to the system to test the repair, air that contained peracetic acid (PAA) residue flowed from the tote filling wan. The employee contacted the PAA mist, resulting in hospitalization for second-degree chemical burns to the face, swelling of the lips and throat, and irritation to the lungs.

Performix Nutrition Systems, LLC

An employee was pouring a copper food additive into a bucket when the product splashed into his eyes and face. The employee also inhaled and ingested the substance. The employee suffered copper poisoning.

Dairy Farmers of America

An employee was rebuilding a valve on a refrigeration system and had pumped down the ammonia in the system to conduct maintenance. He removed a cover and was sprayed by residual anhydrous ammonia. The employee sustained chemical burns to the face and hands and may have sustained a respiratory injury.

Lockheed Martin Mission Systems and Training

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

On June 27, 2025, an employee was walking out of the hangar when he tripped and fell on the concrete ramp. He sustained lacerations to his right elbow and right hand, and was hospitalized with a fractured right hip.

An employee was performing maintenance activities on a ladder when they fell approximately 46 inches to the ground, resulting in a fracture to the left pelvis/sacrum.

Raytheon Company

An employee was loading a 150-pound block of aluminum into a CNC machine when their right hand was struck by the block. The employee sustained a fracture and severed tendon to the index finger, as well as a laceration to the middle finger that required stitches.

An employee picked up material out of a cardboard box that was on the floor. As he was carrying the material, he stepped backward and tripped over the cardboard box, causing him to fall and suffer a compound fracture of his leg.

WADDELL CONCRETE INC.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

Corrigan Brothers Inc

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

Plumb Supply Company

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.
