Final narrative
An employee was using a 3 1/2-pound dead blow hammer to re-crimp the swage of a cable for a metal identification tag. The hammer struck the employee's left index fingertip, causing a crush injury. The employee underwent a partial medical amputation.
While operating a manual end-milling machine, an employee's right hand was caught in the point of operation causing his right middle, ring, and little fingers to be amputated. Also, a tendon was severed in their right forearm. The machine was guarded at the time of the incident.
