An employee stepped onto a steel grate covering a 24-inch by 24-inch, 18-inch-deep area of a longer concrete pit containing heated water. The grate shifted and gave way, and the employee stepped into the hot water resulting in thermal burns to both lower legs.

An employee was removing insulation from an exterior abrasive blast unit following a maintenance event. They stepped into a gap in the floor grating in the interior of the blast unit and their leg was fractured.
On September 27, 2023, an employee was grinding the underside of a railcar using a handheld grinder. The grinding disc split in half and struck the employee on the right leg, just above the knee. The employee sustained a 3-inch laceration and a fracture.
