An employee was installing helical pipe. His left index finger was inside a helical pipe extension when the drive head attached to the extension was activated. The extension spun and the employee's left index finger was caught between it and another helical extension. He suffered an amputation to the fingertip.
An employee was helping a forklift operator move a pallet and a 4X4 piece of wood when the employee's right pinky was crushed between the pallet and the 4x4, resulting in a fingertip amputation.
