Final narrative
On October 14, 2021, an employee was trying to maneuver a powered industrial truck off a dock leveler near warehouse racking. The truck was caught on the dock leveler; as the employee moved it side to side to free it, the employee's left hand was caught between the truck's handle and a warehouse racking column. The employee suffered a crushing injury to the hand, wrist, and forearm (with no fracture or dislocation) and was hospitalized for surgery.

An employee was replacing a linear motor shaft (a highly magnetic object) on a machine. The new shaft was placed on a work table next to the old shaft. Magnetism caused the two shafts to come together quickly and crush the employee's right middle fingertip. The employee suffered a degloving/amputation from the top of the fingernail.
An employee was working to free a conveyor pallet that was jammed under a conveyor. Their right glove got caught in the conveyor and they sustained partial amputations to the index and little fingertips.
At about 12:30 a.m. on July 25, 2022, an employee was performing preventative maintenance on one of the conveyor motors on a manufacturing line. The employee was trying to determine belt tension when the tip of their right index finger was caught between the gear and the belt of the conveyor motor. The fingertip was amputated.
