The injured employee was cleaning the assembly bench workstation at the end of his shift. Another employee used an 8-inch metal scribe to stab the injured employee on the side of the head by the ear.

An employee opened an overhead garage door on a dock. As the employee was looking for IT cables on the outside of the building, he fell off the dock and hit his head on the concrete about 4 feet below. He suffered a skull fracture and brain bleeding.

An employee was completing a sensor replacement on an air-cooled chiller unit and went to sit on a 5-gallon bucket during a rest break. He experienced sudden stiffness in his left knee, causing him to fall backward and strike his back on a pipe hanger. The employee sustained three broken vertebrae in their lower back.

An employee was sitting at their computer when they fell off their chair, resulting in an S4 compression fracture in their spine.

An employee was sitting on a tall teller chair in a bank drive-through. She leaned forward in the chair when it rolled out from under her. As she fell, a metal footstool that was positioned under the teller station was knocked over. The employee fell on top of the footstool and suffered a puncture wound on the right side of her abdomen from one of the metal legs on the footstool.

An employee fell out of a chair, resulting in an injured ankle that required surgery to repair.

A switchboard operator leaned back in her chair and the chair tipped over. She fell backward and struck her head on the floor, sustaining a subarachnoid bleed.

On August 26, 2025, an employee was moving a loaded pallet with a pallet jack. She pulled on the pallet jack to get it unstuck from the pallet. When the pallet jack came loose, she lost her balance and fell on the concrete floor by the loading dock door. She landed on her right hip and it was fractured in three places. She was hospitalized and required surgery.

An employee was replacing a linear motor shaft (a highly magnetic object) on a machine. The new shaft was placed on a work table next to the old shaft. Magnetism caused the two shafts to come together quickly and crush the employee's right middle fingertip. The employee suffered a degloving/amputation from the top of the fingernail.

On June 19, 2025, an employee was using a bearing press when his left little finger was caught in the press. The employee's finger was crushed and partially amputated at the first knuckle.

An employee was working to free a conveyor pallet that was jammed under a conveyor. Their right glove got caught in the conveyor and they sustained partial amputations to the index and little fingertips.

At about 12:30 a.m. on July 25, 2022, an employee was performing preventative maintenance on one of the conveyor motors on a manufacturing line. The employee was trying to determine belt tension when the tip of their right index finger was caught between the gear and the belt of the conveyor motor. The fingertip was amputated.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

A skid loader backed over an employee's leg, breaking it.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
