Final narrative
An employee was walking in a shipping area. On the floor was a 68-inch piston rod to be boxed and shipped out. One end of the rod was propped up on a 5-inch tall box with a protruding piston nut installed on this raised end. Adjacent to the raised piston end was another box containing a part ready to be shipped. The employee stepped around this boxed part with her right foot and then, while attempting to step over the raised end of the 68-inch piston with her left foot, caught her left boot toe on the protruding nut, causing her to trip and fall to the ground. The employee sustained a fractured left humerus.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was spotting a crane when the load (fabricated skid for packaged equipment) that was being picked up came loose and fell on the employee. The employee sustained a crush injury resulting in a fractured pelvis.
An employee was setting up dies on a die casting machine. The machine's ram unexpectedly returned from the down position and caught the tip of the employee's right thumb against a pinch point in the shot sleeve. The employee suffered an open fracture/amputation to the thumb tip. The machine was not locked out at the time.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
