An employee was manually adjusting a wooden pallet when he lost his balance and fell to the floor. The pallet fell on the employee's abdomen and they were hospitalized with internal bleeding.

On July 3, 2024, at approximately 8:00 AM, an employee was installing a pulling sheave in an electrical gear when they received a high-voltage electrical shock and sustained arc flash burns.

An employee was walking down a concrete pier, tripped over a pallet, and fell. The employee sustained a broken arm.

An employee was removing a part from a brake press when the machine's backstop came back and caught the employee's finger against the bottom die. The employee's fingertip was crushed and degloved, and the employee was hospitalized.

An employee was walking when they slipped and sustained an injury to the foot/heel/lower leg.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was operating a proof testing machine by loading it with the product to be tested. The employee's right hand was underneath the door area of the machine and the machine crushed her ring finger and little finger. The injuries resulted in amputation of both fingers up to the knuckle of the hand.

On March 25, 2025, a maintenance employee was elevated 19 feet in a scissor lift to troubleshoot an overhead shipping door that was not fully closing. The overhead door came down and pushed the aerial lift over. The lift struck the concrete floor of the shipping bay and the employee sustained a compound fracture to his left forearm and a fractured pelvis.

An employee was cutting a long piece of wood on a table saw in the maintenance shop. The wood kicked back, causing the employee's left hand to contact the saw blade. The employee sustained an amputation to their thumb tip.

An employee was building a magnet channel assembly. The employee's right middle fingertip was caught between two magnet plates, causing the amputation of about 1/4 inch of the fingertip.

An employee was bending a part on the brake press when his thumb was pinched between the part and the articulating lift, resulting in a crush injury and amputation to the thumb.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
