An employee was lifting traffic control devices and loading them into the bucket of a skid steer when he suffered a hernia. The employee was hospitalized for surgery.

Silver Star Construction Co.

A crew of employees was cleaning out an asphalt burner/drum due to excessive buildup. The burner/drum was de-energized and was being manually turned to allow excessive buildup to fall onto a drag conveyor. Some of the large chunks became lodged on the drag conveyor, and the injured employee used a 5-foot steel bar to knock out the chunks. He reached down with his left hand when another large chunk struck his hand and pinned it between the steel bar and the drag conveyor. The employee suffered an amputation of the index finger and a laceration to the middle finger.

An employee was gathering paperwork to distribute to different offices. When she got up and walked around her chair, her foot got caught on the base of the chair and she fell. She sustained a left pneumothorax, fractured ribs, a scraped left elbow, and injury to her left hip.

An employee was working to string cable at a temporary asphalt facility. He had been taking cable from the silos to the control room. He used a ladder to get to another silo and connect a cable. He slipped on the ladder and fell. A cable-supporting hook next to the ladder impaled and tore his right bicep. The employee required surgery.

An employee was lifting a motor and reaching to place it on a machine when they sustained a back injury. The employee was hospitalized.

On 12/22/2023, an employee was carrying bins filled with pasta to a refrigerator. As he lifted a 20-pound bin, he felt pain in his lower back. The employee sustained a herniated disc.

An employee was lifting a patient when they pulled a back muscle. She suffered back pain and spasms radiating into her leg with weakness and numbness. The employee's L3-L4 disc was herniated and she was hospitalized.

The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Meeco Sullivan

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

Burgess Manufacturing of Oklahoma Inc.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
