On January 29, 2022, an employee was walking through a digester area to return a piece of equipment and have it serviced when he fell about 2.5 feet into an uncovered u-drain. The employee contacted the hot condensate (about 180 degrees) that flows through the u-drain, resulting in second- and third-degree burns below both knees. The employee was hospitalized.

A temporary employee walked across a wooden pallet to retrieve product to stock. When he walked back across the pallet, it broke, causing the employee to fall to the ground and sustain a fractured hip.

A & K Rentals, LLC

On April 10, 2023, at 12:00 PM, an employee was using a broom to clear rock from a building's foundation in preparation for a spray-on vapor barrier. There was a 4-foot deep trench next to the building where the employee was working and the soil at the trench's edge fell away. The employee fell into the trench and was hospitalized for a left pelvis and/or femur fracture.

C&S Enterprise, Inc.

An employee was walking up a steel staircase to check wiring in the electrical room. The staircase had a top platform with a grate that was sitting across the beams of the platform. The grate had shifted and as the employee stepped on it, it gave way, causing him to fall approximately 3 feet to the ground below. The grate landed on the employee and he was hospitalized for four transverse process fractures in the lower spine.

Sams Club

An employee was stocking the freezer and stepped on a wood pallet. The employee's foot went into the pallet and they sustained a right ankle fracture that required hospitalization and surgery.

An employee was loading black liquor into a tanker truck using a swivel arm. The arm caught and pinched his right middle finger as it moved, resulting in a partial amputation to the first knuckle.

Highland Pellets, LLC.

An employee was visually inspecting the drive line for a conveyor. There was an explosion in the conveyor, and the employee suffered second- and third-degree burns to both forearms.

An employee was driving forklift with a load when the forklift hit a guardrail and the employee fell to the ground, resulting in a broken elbow, broken shoulder, and lacerations.

DOMTAR AW LLC

An employee was putting a chain back on a sprocket. The gearbox and sprocket contained two drive chains connected to the lowerator (vertical conveyor) and counterweight assembly. The employee received an alarm indicating that one of the chains had gone slack. While the employee was putting the chain back on the drive sprocket with tongue and groove pliers, the weight of the chain and counterweight fell back in place on the sprocket, pulling the employee's left hand into a pinch point. The employee's ring fingertip was amputated up to the fingernail.

While troubleshooting equipment, an employee tripped on the turntable of the conveyor system and fell to the floor, fracturing their right femur.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
