An employee was cutting a 2x4 with a circular saw and amputated their left index fingertip.
An employee was preparing to replace links in the track of a dozer. As he was setting up a hydraulic press tool and working to get it in the right position, a coupling on a hydraulic hose gave way. The coupling struck the employee's right arm, resulting in lacerations and a fracture.
On August 18, 2024, at 7:30 PM, an employee loaded a milling machine onto a lowboy trailer. While raising the lowboy deck, the welds broke on the trailer and the trailer dropped to the ground, landing on the employee's right foot. The employee's foot was crushed and multiple bones were broken.
An employee was retrieving rigging straps to begin rigging a stack of wooden and concrete forms to be lifted via an excavator boom off a bridge deck. While retrieving the straps, he stepped between the stack of forms and the excavator boom. At this time, the excavator shifted, causing the boom to shift and strike him in the head and neck area.
