An employee was walking across the top of a machine that was being deconstructed. He had been tasked with removing the top of the machine. He fell through an opening on the top, landing on the concrete floor 20 feet below on his feet and hitting his head. He suffered a head injury, a broken ankle and elbow, and a broken knee.

An employee was transitioning from a ladder onto a roof when the ladder slipped and they fell approximately 20 feet to the ground. The employee was hospitalized with fractures to their pelvis and heel.
