On April 6, 2022, an employee was crossing a new road in a vibratory roller to compact the material on the new road when the roller fell off the road and into an adjacent canal. The employee suffered back pain and a laceration to his inner left thigh.

An employee was releasing a chain to open the rear flap of a dump truck when three of their fingers got caught between the chain and the truck, resulting in crush injuries.

A front-end loader had been flipped over upright, and employees were getting ready to unload its bucket. When pressure on the hydraulic lines was released, the bucket lowered quickly and crushed an employee who had walked beneath it. The employee suffered three broken upper-back vertebrae and a punctured lung.

An employee was dislodging a rock from the conveyor belt of the rock crusher using a piece of rebar/pipe when the employee's right hand slid into a pinch point between the frame of the equipment and the rebar resulting in a partial amputation of the right index fingertip.

On February 7, 2019, a dump truck an employee was driving broke down and was placed on a flatbed. The dump truck driver climbed onto the flatbed and inside the truck cab to retrieve his lunch box and upon climbing down he fell to the ground below and struck his head. The employee was hospitalized for a head laceration requiring stitches.

An employee was repairing a dump truck on the side of the road when his finger was caught in the pulley and belt of a compressor. He suffered a partial amputation of the left ring finger at the tip, after the nail.

An employee was operating a sit-down type of material handling equipment (MHE). They were turning the MHE with the clamp attachment in the air when the MHE fell over onto its right side. The employee's right foot was caught under the battery cover of the MHE. The employee sustained a chemical burn from the battery and a fractured right ankle.

The employee was walking between a forward-moving dump truck and a dirt pile when his right foot was run over by the truck s wheel, causing him to fall. The employee sustained injury to his foot.

An employee was operating a skid steer with a bucket attachment at a demolition site. While driving it to the rear of the property to park it, the employee came into contact with a live electrical transformer unit that was located outside of the facility. The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
