Employees were building a water pool for a pump site. Just after being handed a rock to stack, an employee lost his balance and fell forward while holding the rock. His finger was crushed between the rock he was holding and another on the ground, and he suffered an amputation.

An employee was inside a ditch while pull-starting a gas-powered water pump with their left hand. As the employee pulled the handle, their hand was punctured by a stick protruding from the bank of the ditch, resulting in a wound that subsequently became infected and required hospitalization.

An employee slipped and fell while working in steep, rough forest terrain. A sharp stick or other object pierced the employee's clothing and lacerated the employee's abdomen.

An employee was making a pride loop to tie a horse. The horse got aggressive and the loop closed around the employee's left hand amputating the fingertips.

An employee responded to a wildland fire and was constructing a line. The employee sustained heat stroke and was hospitalized.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was on a four-step work stand while sanding an area of an aircraft. He was climbing down the work stand when he tripped over an air hose and fell to the level below, resulting in a fractured femur.

An employee was lifting a manhole cover when it fell on his right foot, resulting in the amputation of his big toe and the adjacent toe.

A Bureau of Land Management (BLM) employee was assisting a fire crew with clearing a forest trail. The employee was struck in the left leg by a fire-rated chainsaw. The chainsaw struck the employee underneath their chaps and above their boot. The employee was hospitalized and required surgery.

An employee handed a fuel hose to another employee at a gas station pump and then turned around, tripped over the hose, and fell to the ground. The employee was hospitalized with a fractured hip.

An employee was conducting fire suppression operations on a wildfire. He pulled a hose from the engine. He then charged the hose and began to put on his gloves. A flareup occurred and the employee sustained burns to his face and hands.

An employee was servicing an exterior mounted environmental control unit while on a 10-foot ladder. He pushed through a pre-made knockout hole on the panel. Upon bringing his arm down, a sharp area along the panel frame lacerated the top of his hand. The employee required surgery.

On 7/31/2025, an employee was training to ride an ATV when it went into a draw and she went over the front of the ATV, landing on the ground. The employee sustained a fractured arm.

An employee was standing behind a frac pump. A discharge hose failed and the employee was burned by hot water on the left upper abdomen. The employee was hospitalized.

An employee working as a contracted firefighter was using a bulldozer to push back an active fire line when they became entrapped in flames, resulting in first- and second-degree burns.

An employee was assisting with the delivery of pipe racks to the drilling rig. The driver was releasing the tie-down lock straps that were holding the pipe racks on the trailer. When the last strap was released, one of the pipe racks rolled off the trailer and struck the employee while they were on the passenger side of the trailer bed. The employee sustained an open wound to the hand and fractures to their left leg and spine.
