An employee was handling a 1/32 drill bit sleeve. He pressed the sleeve cover closed and the drill bit hung on the lip inside. This caused the drill bit to puncture the case and his right thumb. At that point the drill bit broke off and a piece remained inside his thumb.

The safe fell from the dolly, struck the ground, fell forward onto the employee, striking their right hip/pelvis area before rolling off the employee onto the floor.
