Final narrative
An employee moved the equipment that pours molten metal into molds away from the access platform so that the overhead jib crane could remove the plug on top for cleaning. When he attempted to jump back to the access platform, which was approximately 39 inches away, his open fire-resistant shirt caught on the guardrail gate, causing him to fall approximately 14 feet to the floor below. He suffered broken ribs and bruises and was hospitalized.

An employee was using a hydraulic hoist to lower a 200-pound manhole cover into a frame so a safety hinge could be installed. The lid became dislodged and fell from the hook, landing on the employee's fingertips. He suffered an amputation to the left middle fingertip, as well as an avulsion to the left ring fingertip.
An employee was using a pencil grinder equipped with a cutting wheel to enlarge an opening for an LCD display in a steel access door. The cutting wheel shattered and a fragment struck the employee in the face, resulting in a laceration to the right cheek and a fractured tooth. The employee was hospitalized.

While the injured employee entered the auger area, the second employee went to get a pair of pliers to tighten the wing bolts used to secure the leather chute to the auger. The second employee returned with the pliers and was given the leather chute, which had been removed by the injured employee. The second employee was using the pliers to clean the cute and remove material that had hardened inside. The injured employee was attempting to push the auger arm out of the way when they slipped and their hand entered the chute opening. The
injured employee's left hand had made contact with the auger, which was still in operation, and resulted in a partial amputation to the index and little finger with a total amputation of the ring and middle fingers.
