A chemist was finishing a reaction. He added paraformaldehyde to a Grignard solution in a 5-liter glass jacketed reactor. The reaction mixture over-pressurized and the reactor shattered, spreading shards of glass throughout the room. The chemist sustained lacerations to the chest and thigh.

Freehold Cartage Inc

An employee was sitting in the cab of the truck while waste oxidizer was being offloaded. The waste reacted with the receiving tank and an explosion occurred in the tank. Fire traveled through the hose to the tanker truck, causing it to explode. The employee jumped from the burning truck and was hospitalized with burns to his face, chest, and arms.

Complete Fire Protection Inc.

An employee was installing piping and valves for fire protection systems in the vicinity of an electrical meter outside of a new warehouse. The meter exploded, causing burns to the employee's face and arms.

MI Metals, Inc.

An employee was pouring molten aluminum into a drain pot. The aluminum touched moisture at the bottom of the drain pot and an explosion occurred. The employee sustained burns to their face, hands, and legs.

An employee was operating an oxygen-deficient oven that required manual ignition to make charcoal from wood. As the oven was opened, oxygen was introduced to the environment, resulting in an explosion. The employee sustained head trauma as well as fractures to the arms and legs.

RKM Fireworks Company

An employee was setting up the electrical parts for a fireworks display. They were inserting an e-match when it generated friction with something else. A 3-inch firework shell ignited, injuring the employee's hand and head.

An employee was using a hand tool to cut a mooring line at a marina to demonstrate equipment to customers. The hand tool broke and struck the employee in their left eye, lacerating the cornea.

An employee had just gone through a roll-up door and closed it. The rope used to pull the door down caught her foot and she fell, breaking her left kneecap.

An employee was changing the bottom die on a hydraulic power crimper. Their left thumb was crushed between the two die components when the crimper engaged, resulting in thumb fractures.

At 8:50 a.m. on October 6, 20201, an employee was positioned behind a metal shear to help hold a piece of material about to be cut. As the shearing machine was engaged, its vertically moving backstop pinched the employee's left arm against a table below, in place to catch clippings. The employee suffered broken bones just above the wrist and was hospitalized.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
