An employee was descending the back stairwell to go to lunch when he missed a step. As he fell forward, his toes hit a step which pulled his right leg behind him. He landed on the concrete stairwell landing between floors. He sustained a partial rupture of the right quadriceps tendon and an avulsion fracture involving the right patella. The employee required surgery.

An employee was driving a 4-seat ATV from one field to the next field on the site. They went around a corner and glare from the sun blocked the employee's sight. The ATV struck a parked car. The employee sustained a broken hip and a laceration to the head.

An employee was talking to a patient when he lost his balance and fell down the stairs. The employee sustained injuries to both patellar tendons and required surgery.

An employee was helping a patient get out of bed and adjusting the bed rail when the patient became agitated and kicked the employee in the abdomen. The employee was hospitalized.

At about 4:00 p.m. on March 8, 2024, an employee was using a table saw and his hand contacted the blade, resulting in amputation of the tip of his thumb.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee collected rags in a housekeeping closet. They were finishing up their shift and traversing to a locker room with a wheeled garbage cart. The employee fell to the floor and struck their right hip. The employee was hospitalized with a fractured hip.

An employee was getting up from her chair when she twisted her left ankle and then injured her right foot while falling. The employee was hospitalized with a closed displaced trimalleolar fracture of the right ankle.

An employee was setting chains to secure a concrete barrier to a front-end loader. The chain became twisted and lost slack. The concrete barricade fell on the employee's left foot, causing a partial amputation to a toe.

On December 2, 2024, an employee was exiting a patient room while her foot was entangled in the wires of a sonograph machine and she fell to the floor. The employee sustained a non-displaced fracture to her left femoral neck.

An employee was cleaning a daily room. She walked in and took trash out from the trash can. She then took a step back and her right shoe got stuck/stayed gripped on the floor, causing her to fall on her right side. The employee was hospitalized with a fractured right hip and required surgery for her right wrist.

On September 25, 2025, an employee climbed an aluminum extension ladder and was on top of the roof of a building She was walking around on the roof, inspecting and evaluating it. She was descending the ladder when the ladder slid to the right and twisted around as she hung onto it. She then fell 10-12 feet to the paved parking lot and the ladder landed on top of her. She sustained a fractured left collarbone, multiple other fractures, and had bruising to the left side of her abdomen.

At about 5:55 p.m. on August 9, 2025, an employee lost his footing while climbing an access ladder to a 12-foot-high spotlight platform. He fell about 9 feet to the ground, breaking eight ribs on the right side.

On August 1, 2025, an employee was preparing to initiate canine training when they were bitten by a dog on the neck and face.

An employee was completing an annual work capacity test. Later that night, the employee experienced severe pain in their left leg and was hospitalized with rhabdomyolysis and compartment syndrome in the lower left leg.
