An employee was operating a structural mill when a line became jammed between the cut punch system and the roll former. The employee was guiding the material back onto the roller when their right hand contacted the cut punch, resulting in partial amputations to the index, middle, and ring fingers. The area was unguarded at the time.

A maintenance employee was working on a structural line when his right middle finger was caught in the belt and belt pulley and the fingertip was amputated. The machine was not locked out at the time.

On August 13, 2022, an employee was going to lift a 3,000-pound steel coil with an overhead crane. When the employee tried to move a wood coil spacer, the coil shifted and crushed his hand against another coil.

The employee's arm was fractured at the elbow and required hospitalization.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
