On 11/22/2023, at 8:35 AM, an employee was stepping onto a barge when they slipped on a wet surface and fell on their knee. The employee sustained a torn ligament in their knee requiring hospitalization and surgery.

After moving a barge lid, an employee was walking down the stairs on the barge when they slipped and fell approximately 15 to 20 feet, reuslting in a head injury.

An employee was using an excavator on a barge to scoop and remove fertilizer product stored in the cargo area. As the employee was transporting the skid-steer by chains and hoisting equipment from the dock to the barge, the employee fell approximately 10 feet off the skid-steer bucket to the cargo hold area below. The employee suffered a concussion.

Pacific Terminal Services Company

Swingmen were performing discharge of an iron ore vessel. The discharge process consists of gathering remaining product into the center of the hold to facilitate removal by crane. At approximately 11:45 PM, a swingman was descending a ladder into a hold when he lost his footing and fell approximately 10-15 feet to the deck of the hold. The employee sustained fractures to their back and left leg.

JONES STEVEDORING COMPANY

An employee was on a bulk carrier ship. While removing lashing from wind turbine components, the employee lost balance and fell approximately 6 - 7 feet from a hatch to the deck of the ship. The employee was hospitalized with a head injury, and a fractured wrist, and required surgery.

He stepped on an unsupported end of the sheet metal and the sheet metal folded, causing the employee to fall approximately 15 feet to the cement floor. The employee sustained fractures to his wrist, hip, and femur.

An employee was opening the door of a container. His finger got caught between the container doors, resulting in a fingertip amputation.

An employee was removing bailing wire from the floor. As they bent down to pick up the wire, a strand of bailing wire that was sticking up from the floor went under his safety glasses and struck his eye.

Greggo & Ferrara, Inc.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

GREGGO AND FERRARA, INC.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.
