An employee was bending down, reaching through a tower frame for a work platform to bind straps and frames together in preparation for shipping. The frame flipped over and landed on his left ankle, dislocating and fracturing it. He was hospitalized.

An employee was dismantling scaffold that was three frames high and had planking that was three planks wide. As he was dismantling the ladder portion, the ladder tipped forward and he lost his balance and fell 18 feet to the ground, suffering a left heel fracture, a right ankle fracture and compression of numbers 1, 2 and 3 vertebrae.

The employee stayed inside the scissor lift basket as he fell to the floor. The employee sustained fractures to the right arm and hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.
