A 370-pound piece of aluminum was being lifted with a jib crane and a calibrated suction lifting fixture. The aluminum fell, landing on an employee's left foot about 3 feet below. He was hospitalized; his big toe had been broken, and the other four toes were medically amputated.

On June 27, 2023, at 2:30 p.m., a pilot was returning after a test flight of an F-35 aircraft. After aircraft recovery, the pilot exited the aircraft via the integrally mounted aircraft access ladder. During his descent, he tripped on the third ladder step and fell approximately 6 feet, resulting in a dislocated left shoulder, a compound dislocation of the left elbow, and stable fractures of the left 7th and 8th ribs.

An employee was using a clutch spreader tool during a training activity. When he applied pressure to the tool to release belt pressure, his left index finger came in contact with a spring sheave roller. The finger was amputated just below the nail.

An employee was in a final assembly building, seated on a workbench chair with wheel casters, working on equipment wiring. The chair slid out from under the employee, who fell about 2 1/2 feet to the concrete floor and suffered a broken hip and right femur.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

KP Building Products, Inc.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

BJ'S WHOLESALE CLUB, INC.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

Seneca Mechanical, LLC

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

On June 27, 2025, an employee was walking out of the hangar when he tripped and fell on the concrete ramp. He sustained lacerations to his right elbow and right hand, and was hospitalized with a fractured right hip.

An employee was performing maintenance activities on a ladder when they fell approximately 46 inches to the ground, resulting in a fracture to the left pelvis/sacrum.

An employee was loading a 150-pound block of aluminum into a CNC machine when their right hand was struck by the block. The employee sustained a fracture and severed tendon to the index finger, as well as a laceration to the middle finger that required stitches.

An employee picked up material out of a cardboard box that was on the floor. As he was carrying the material, he stepped backward and tripped over the cardboard box, causing him to fall and suffer a compound fracture of his leg.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
