A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was removing decals off the side of a tanker truck while on an 8-foot step ladder. He had one foot on the third rung of the ladder and one foot on the hose tray of the truck. The ladder kicked out from underneath the employee. His feet were tangled in the ladder as he fell approximately 3 feet to the ground. The employee sustained a fractured pelvis and required surgery.

The truck struck him and when he went to roll out of the way, the trailer ran over his left leg. The employee was hospitalized with a fracture and puncture to his left leg and road rash to both of his legs.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
