An employee was removing a governor from an elevator system with the govern rope suspended from the sheave. A co-worker grabbed the suspended cable to move it out the way when it slipped through the cable grab and pinched employee's right middle finger between the cable and the sheave, resulting in an amputation at the distal interphalangeal joint.

An employee was installing a new elevator, working from a false elevator car in the shaft. A railing on the false car fell apart and the employee fell through it, landing 23 feet below in the pit of the elevator shaft. The employee suffered a broken foot, dislocated shoulder, and broken pelvis.
