An employee was loading 55-pound ballast bricks into a space on a submarine with help from a co-worker. The co-worker passed a brick to the injured employee when it fell onto the employee's left ring finger, pinching it against a steel bin. The employee sustained a compound fracture of the left ring finger.

An employee had been performing abrasive blasting operations in the sonar dome of a submarine. The employee completed replacing the outer blast glass layer on their blast hood and, while kneeling, the dead-man switch on their blast hose was actuated. High pressure (125-130 psi) air and blast media struck and injured their left forearm and hand. The employee was hospitalized and required surgery.

An employee was performing paint spraying operations when he experienced an issue with the spray gun. He twisted the spray tip back and forth to free some stuck material. The trigger assembly was actuated, causing paint to be injected into his left thumb. The paint entered into his bloodstream.

An employee was performing abrasive blasting operations within a confined space onboard a naval vessel when the end of the abrasive blast hose slipped from his hand. The employee went to catch the abrasive blast hose with his right hand, missed the end of the hose, and caught the end of the abrasive blast hose in the bend of his right elbow. The abrasive material caused lacerations and contusions to his inner arm. The employee was hospitalized and required surgery.

Four employees were lifting a 10-foot by 10-foot metal gate. One employee's right middle finger was caught under the gate, causing the amputation of the fingertip.

An employee was using a grinder operating at 5,000 rotations per minute (RPM). The grinder also had an offset deck crawler-scarifier attachment with an enclosed vacuum shroud and a built-in handle. The employee stepped on the trailing power cord, began to fall, and lost his grip on the deck crawler-scarifier, which was approximately 15 pounds. The tool's rotating head contacted the adjacent metal rail, causing the deck crawler-scarifier to flip onto its backside exposing the rotating scarifier teeth. When the employee attempted to brace himself using his left hand, his glove contacted the scarifier teeth, causing severe lacerations to his left index and middle fingers and partial degloving of his left ring finger. The severity of the injury resulted in the partial amputation of the left index, middle, and ring fingers.

An employee was bracing an extension ladder while his coworker installed a 10-foot, 40-45-pound pipe. The pipe fell onto the injured employee's head and he sustained a hematoma, C3 and C4 vertebral fractures and a forehead laceration.

An employee was helping to lift a press roll in to fix an issue that was occurring. A co-worker lost their grip and the press roll fell onto the injured employee's left index finger, resulting in a crush injury and amputation.

VIS - Vega's Industrial Services, LLC

Three employees were removing equipment from an old fermentation room at a bakery. They were using two bakery dollies to remove metal plates from the floor of the room. One of them was holding one of the metal dollies with his right hand on the side of the dolly, as a plate was being centered on the two dollies. The plate slipped and struck his right thumb, which was amputated near the last joint.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee was preparing to bend a flat bar in a brake press. When the machine was jogged, the stock rotated up and crushed his left middle fingertip against the outer frame of the die. The fingertip was amputated.

An employee was cutting metal with a torch. A piece of metal struck the employee's left foot, causing multiple fractures to metatarsal(s). The employee was hospitalized.

An employee was descending a ladder carrying a bag of trash. He fell, landed on the ground about 10 feet below, and suffered fractures to his right hip and pelvis.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

An employee was walking back to a carrier case with mail in her hand when she tripped over a tub. Her back overarched as she fell, resulting in a fractured back that required hospitalization.
