An employee was changing the motor on a four-bin feeder at the asphalt plant. A small backhoe was parked on ground that was sloped towards the feeder bin and an outrigger had been extended as a safety stop. The employee stepped into the bucket and weight was transferred to the front of the backhoe allowing the outrigger to come off the ground. The backhoe then rolled forward. The employee got caught between the backhoe and the feeder bin and sustained bruising to the upper leg.

An employee was inspecting a hopper bin when their loose clothing was caught in a tail pulley. The employee suffered a broken arm.

An employee was working on the brake pads underneath a broom (equipment vehicle) when something fell and struck the employee. The employee suffered a broken right arm and was hospitalized.

The employee was in a front end loader attempting to lift a railroad tie. She lost her grip on the railroad tie and it fell striking her right hand, middle finger on the bucket blade. Her right middle fingertip was amputated.

An employee connected a steam line to a hose to clean equipment when the fitting broke loose. They were struck by steam in the left inner thigh, resulting in burns that required hospitalization.

An employee was straining hot water from a pot of rice when the water splashed onto them, resulting in burns to their chest, arms, shoulder, and back.

An employee was making tea when she noticed tea grinds were collecting on the side and water was no longer dripping through the funnel. The employee was checking the funnel when boiling water and tea grinds spilled onto the left side of her body. The employee sustained burns to her neck, back, and arm.

An employee had turned off the ball valve on a waterpipe system and was removing the plug when the coupling system attached to the strainer came apart. Hot water sprayed on his arm and back, resulting in first- and second-degree burns that required surgery.

An employee was using a shovel to remove waste vermiculite from molten zinc. The metal had been placed in a bin and partially hardened. The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
