An employee was securing two pipes on a band saw. His right hand was placed between the pipes when the pressure switch was activated. The two pipes rolled together and the employee sustained an avulsion to right index finger, amputation of the right middle fingertip and distal finger fracture.

An employee was using a crane to move a pole through the squaring press for the third pressing operation. The attachments used to support the pole fell. The top attachment, the die, which supports the knuckle of the pole and weighs approximately 125 pounds, stuck the employee's ankle just above his metatarsal guard. The employee sustained a right ankle laceration requiring surgery to repair a tendon.
