An employee was cleaning an industrial roller while wearing gloves when his hand and fingers were pulled into the roller. While attempting to pull his hand out of the roller, the employee sustained a finger amputation and a hand injury.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.
