An employee was driving a cart around the driving range to pick up golf balls. The vehicle struck an in-ground target causing the employee to strike his forehead on the windshield. The employee was hospitalized with fractures to their nose and cheekbone and lacerations to their head and hands.

An employee was operating a picker when it struck a stack of pallets. His body moved forward and he struck his face on the computer monitor in the vehicle, resulting in a concussion, laceration, and blurry vision.

An employee was driving a lift in reverse through an aisle while the lift was loaded with a headboard. The headboard caught merchandise in the aisle racks, causing it to shift and contact the employee's right leg. The employee suffered a broken leg.

An employee was driving a utility task vehicle (UTV) through the parking lot to the boat yard to meet a co-worker. The UTV and slid into a curb, causing it to flip on its side and land on the employee's leg, resulting in a broken leg.

On December 7, 2023, an employee was driving an order picker down an aisle when the mast of the vehicle struck a material pan that was hanging out of the rack. The pan fell and struck the employee, resulting in rib fractures and a spleen laceration.

An employee was positioning a part inside a restrike hammer (full revolution drop hammer). The employee was wearing his safety pullout devices (metal cable with wristlets) at the time. As the employee positioned the part inside the die, he tripped the machine with his palm. As the ram went to drop, his pullout device (cotton rope connection) broke away from the press and his right hand was crushed by the top die as it fell. The employee was hospitalized and required surgery.

A piece of sheet steel fell from a sawhorse onto an employee's right leg and foot, resulting in fractures.

An employee was removing the bottom die from an auto hammer. He was pushing the die out with his left hand when the die bar slipped and fell on top of his left index finger, resulting in a fingertip amputation.

An employee was troubleshooting a coolant door fault on a machine when the right door actuated and crushed his right index finger, causing an amputation to the finger.

At about 9:00 a.m. on March 10, 2025, an employee was operating a lathe when a part jammed inside it. The employee was removing the jammed part when the machine's slide shifted, causing the lathe spindle to amputate his left ring fingertip.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
