Final narrative
An employee attached a sling to a bridge crane and then to the forks of a forklift. The employee then used the forklift to lift a steel beam rigged to the bridge crane. The beam shifted and fell on the employee's left foot, resulting in a laceration to their big toe that resulted in an amputation.
