Final narrative
On April 28, 2023, at approximately 9:05 AM, an employee was helping to steer a mobile paper cart when they fell onto the floor. The employee sustained a fracture to their right hip that required hospitalization.
Three employees were engaged in horseplay when one employee suffered a laceration to the abdomen, caused by a pocket knife. The employee was hospitalized.
On November 4, 2023, at 6:08 p.m., an employee was attempting to dump out scrap material from a hopper that was lifted off the ground by a forklift. As they went to unlatch the latches on the hopper, their left hand was crushed between the hopper and the forks of the forklift, resulting in an amputation of the middle finger.
