Three employees were engaged in horseplay when one employee suffered a laceration to the abdomen, caused by a pocket knife. The employee was hospitalized.

On November 4, 2023, at 6:08 p.m., an employee was attempting to dump out scrap material from a hopper that was lifted off the ground by a forklift. As they went to unlatch the latches on the hopper, their left hand was crushed between the hopper and the forks of the forklift, resulting in an amputation of the middle finger.

An employee was setting up slitter knives to be sharpened on a knife grinder when their right middle finger was crushed between a level used to align the grinding wheel and the grinding wheel guard. The employee sustained amputation of their right middle finger between the first and second knuckle.

An employee was performing maintenance on a slit line using an Allen wrench. The slit line suddenly energized and a clamp came down, pinching and bruising both of the employee's forearms. LOTO was not performed at the time of the incident.

An employee was walking in a store when she tripped and fell to the floor. The employee sustained rib fractures.

An employee was walking through a hallway when they tripped and fell to the ground, resulting in a fractured left hip and femur.

An employee was loading a vehicle for mail delivery when she turned, tripped, and fell in the parking lot. The employee struck their head on the pavement and suffered a concussion.

The injured employee had been talking with another employee. As they went to turn around and walk, they tripped and fell to the floor. The employee was hospitalized with a broken hip.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

An employee was removing a roll (tooling) from a machine when their left little finger was pinched between the roll and the machine, resulting in a partial amputation.

An employee was rolling a piece of steel in the belt of a shape roller machine when his finger was pinched between two pieces of steel. His right ring and middle fingers were amputated.

An employee was cutting a length of steel coil when he noticed the steel might have cross-break defects. He used his gloved hand to feel for and clean the defected spots when his glove got caught in the measuring wheel, which rolls along the top of the steel to ensure accurate cuts. The employee's arm was pulled down into the machine until his body was stopped by a support bar that goes across the machine. The employee's arm was fractured at the elbow and required hospitalization.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
