An employee was searching for pieces of a broken ball check valve around the rotating filler valve when the valve closed on their fingertip. Their left index finger was amputated. The machine was guarded at the time.

An employee was walking between two buildings when they slipped and fell on newly formed ice caused by a fire pump that was discharging water after a sprinkler was activated. The employee suffered a contusion to the back of their head.
An employee was removing a flexible hose from a production pipeline. The hose end detached from the pipeline end and sprayed the employee with hot water. The employee suffered burns to the left and right wrists and the front of the leg.
