An employee was operating a pallet jack and staging a pallet of product on the floor next to storage racking. The employee backed the pallet jack out of the pallet and was turning around in the aisle when the pallet jack impacted the racking. The first level of the racking was above the operator's compartment of the pallet jack. Upon impact, the employee was pushed forward within the operator's compartment and pinned against the racking. The employee sustained a fractured sternum.

The employee was struck by the falling pallet of 30-pound cardboard boxes filled with poultry products and he sustained fractures to his right tibia and fibula that required surgery.

The injured employee noticed excess grease on the drive system and wiped it off with a rag while the machine was turned on and running at a slow speed. The rag was caught and the employee's hand was pulled into the drive system resulting in a partial amputation of his right index finger.
