An employee was covering a bucket hoist on a work truck. He was pulling a string to tighten the cover when the string broke and he fell from the work truck platform. He struck his right leg on a vehicle parked next to the work truck and sustained a fractured tibia.

A digger derrick operator was positioning the boom off the back of a truck. He got out of the operator's chair and stepped onto the walk platform to the back of the truck to get a better view of the placement of the anchor and to instruct the ground man. He lost his footing and fell off the back of the truck resulting in a broken left elbow, a broken right wrist and facial lacerations.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
