The injured employee was standing by a pallet adjacent to a storage rack when their right foot and ankle were struck by a center-rider double pallet jack. The employee's foot was crushed between the pallet jack and a wooden pallet. The employee sustained a fractured ankle and/or foot.
An employee was dismounting a pallet jack. Their left hand was on the throttle to help balance themselves. As they got down, the pallet jack drove into their leg, resulting in an injury to their Achilles tendon and hospitalization.
