An employee was working on a truck. He had a small amount of flammable brake cleaner on his arm, and when he was lighting his handheld torch, the flame ignited the brake cleaner. He was hospitalized with burns to his forearms and hands.

An employee was entering a service department from outside. He slipped on ice and landed on his back, also hitting his head.

An employee was under a truck using an impact wrench to remove the first of two bolts located on the carrier bearing of a driveline. When he removed the first bolt, he noticed the second bolt was stripped, but still needed to be removed to successfully remove the entire driveline. As he tried to remove the second bolt, the driveline shifted toward him in a downward motion, causing him to react and catch the driveline with both his hands and feet. He then asked a coworker to help remove the second bolt. Once his coworker removed the nut and bolt, the driveline became loose and fell on his right elbow, smashing it to the concrete and causing a fracture.

On December 26, 2023, an employee went out to the parking lot to troubleshoot the starter of an SUV. The employee was underneath the vehicle when it started and moved and the employee's middle back area was caught under the front passenger-side tire. The employee sustained a fractured collar bone.

An employee was parking at a patient's home and exited the van to manually open the garage door. The van began rolling backward and rolled over the employee's right leg. She was hospitalized with a fractured leg.

A driver exited their truck so they could provide service for a customer when the truck began to roll away. The driver went to re-enter the truck but fell and was struck by the rear driver-side tire. The employee sustained fractures to their leg and foot.

An employee had been operating a yard tractor. He parked the vehicle and was exiting when the vehicle began to roll, causing him to fall. The employee's hand was then run over by the yard tractor, resulting in fractures to multiple fingers. The employee was hospitalized.

An employee was checking for a leak in the air brake system of a semi-truck. The employee had removed the brake chamber when the truck rolled back. One of the tires pinned the employee's right hip against the ground.

An employee was on a 20-foot ladder cleaning tile on the side of a building. The ladder slipped and he fell to the ground. He landed on his feet, resulting in hospitalization with fractures to both feet.

An employee was putting away stock in the aisles and began driving his order picker down the aisle with a pallet of items loaded on the forks. The pallet struck an upright of the warehouse racking and was pushed toward the employee, causing him to fall. The front edge of the pallet drove his right leg into the front panel of the order picker, fracturing his right leg above the ankle.

An employee was completing an inspection on a truck when the flywheel amputated their fingertip.

The injured employee was vacuuming out the trunk of a parked vehicle. Another employee was operating a vehicle when the brakes failed and the vehicle struck the back of the injured employee's legs pushing him into the vehicle he was vacuuming. The employee sustained a fractured leg.

An employee got out of his truck and closed the door when his right hand got caught in the door, resulting in a broken ring finger and an amputation of the middle fingertip that required hospitalization.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
