An employee was working on a truck. He had a small amount of flammable brake cleaner on his arm, and when he was lighting his handheld torch, the flame ignited the brake cleaner. He was hospitalized with burns to his forearms and hands.

An employee was working on a semi-tractor trailer and caging the brakes when the weight shifted. This caused the truck to roll over the wheel chocks and pin his left leg under the front drive tires. The employee sustained a left ankle and tibia fracture that required surgery.
An employee was under a truck using an impact wrench to remove the first of two bolts located on the carrier bearing of a driveline. When he removed the first bolt, he noticed the second bolt was stripped, but still needed to be removed to successfully remove the entire driveline.
