Federal OSHA recorded a severe workplace injury at US Truck Body, 1705 N. Shabbona St, STREATOR, ILLINOIS 61364 on — Amputations, affecting the Finger(s), fingernail(s), n.e.c..

An employee was setting up a sheet of 0.04-inch aluminum to cut on a shear. As he got close to the end of the sheet, he pressed the foot pedal to make another cut, which activated the material holders. The holders crushed the employee's right middle and ring fingers resulting in fractures and amputations down to the first knuckle. The employee was hospitalized. The machine was guarded at the time of the incident.

US Truck Body

An employee was guiding the front of a cart that was carrying a truck body across a service pit while two other employees pushed from the rear. The employee was holding the tongue with a hitch attachment. As he ascended the stairs, his left little finger was caught between the hitch and the side of the top step. The employee's little finger was severed between the first and second knuckle.

An employee was operating a metal rolling machine when their hand was caught and crushed. The employee suffered fractures to the ring finger and amputations to three other fingers.

An employee's right ring finger was caught between a 2X4 exciting the cutter head and the back of the cutter head guards, resulting in a partial finger amputation.

Berg Pipe Mobile

An employee was working as a paint inspector and was inspecting painted pipe for production runs. She pressed the joystick to move the pipe into the number 1 position and then stepped up onto a platform to inspect the end of the pipe. The pipe began to move into the number 2 position that is across the platform the employee was working at. As the employee realized the pipe was moving she tried to back up but was pinned against the pipe roller on the other side of the platform. The pipe continued to move across the platform crushing the employee. The employee suffered a fractured back.

An employee was clearing product from the blade of a saw and her hand was thrown into the blade. The employee's left thumb tip was amputated. A guard was in place at the time.

Deufol Worldwide Packaging, LLC

A temporary employee was cutting short pieces of wood on a pneumatic upcut saw when the blade caused amputations to his right middle, ring, and little fingers. The machine was guarded at the time.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
