An employee was repairing a CNC machine. A tool fell from the operator platform into the pit area below, a distance of approximately 3.5 feet. The employee descended into the pit using the access stairs to retrieve the tool. The employee's feet slid out from under him while stepping onto the cover, resulting in a fall and a fractured left hip that required hospitalization.

An employee, using a sling and hoist, was flipping a liner plate to inspect the underside. When he flipped and laid it down, the plate began to slip across the table. The employee used his right hand to try and stop the plate from sliding and the plate pinched his fingertip against the frame of the table. He suffered an amputation of the last digit of his right middle finger.

As the employee attempted to push the manifold from the first stand, the weight of the manifold became unevenly distributed causing the manifold to fall from the pipe stands. The employee's finger was caught between the pipe stand and the manifold resulting in the amputation of the employee's first digit of the right thumb.

Once the filter dryer was completely disconnected from the support stand, the unit became top-heavy and tipped over onto the employee causing a fracture to their right foot and lacerations to their upper right arm and upper back.
