An employee was standing on top of a 5-gallon bucket while attempting to evaluate a problem with a piece of machinery. The employee attempted to step onto the machinery when he lost balance and fell approximately 18 inches to the concrete floor, resulting in a fractured left elbow.
The injured employee was assisting another employee, who was operating a punch press. The press was actuated as the injured employee was pulling out a part, partially amputating their right index finger.
An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.
