Final narrative
An employee was adjusting a mechanical press and using a prybar on the shaft to reset the cutter and crankshaft to the home position. The employee went to move the shaft, but it caught the flywheel with the momentum. This caused the shaft and prybar to spin around. The prybar struck his left middle fingertip and amputated it.
