On February 21, 2024, employees were pouring molten metal into a horizontal casting machine while the machine was spinning. The front plate and metal mold disengaged from the casting machine. Molten metal was released from the mold throughout the babbitting room. The front plate went flying and struck employee 3 in the head, resulting in a fatal head injury. The front plate also struck employee 1 and fractured their leg. Employee 2 was struck by molten metal and sustained second-degree burns on the face, shoulders, back, and inside an ear.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.

An employee was working to assess the operation of a high-pressure water nozzle. The water system was activated and the employee was cut by high pressure water on his left hand where the palm meets the wrist. The employee was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.

An employee was sandblasting paint from a water tower when the blasting media sprayed across their forearm, resulting in an elbow injury that required hospitalization.

UPS Industrial Services, LLC

An employee was positioning dunnage on a trailer while pipe was unloaded from a forklift. A pipe fell and the employee's right thumb and index finger were caught between the pipe and a piece of dunnage. The thumb was crushed, and the index fingertip was amputated.

Phoenix Tube Company, Inc.

An employee was using a lifting sling on a crane to move a straight-bar bundle and place it on top of an existing bundle. When moving the lifting sling, it became pinched between the bundle and a wood cleat. The bundle then fell onto the employee's left foot, resulting in crush injuries.

An employee was stacking metal tubes using a forklift. A stack became unstable and the employee got out of the forklift to steady it, instructing his helper to get into the forklift. The forklift movement caused some tubes to fall and strike the employee, resulting in an open fracture of his left hand.

An employee was setting up a flexible gear coupling hub in a lathe, preparing to bore it out to fit a shaft. The hub assembly was not secured in the chuck when the crane support was removed, and the employee's right little finger was caught between the hub assembly and the inside of the lathe. The fingertip was amputated.

An employee was using a crowbar to loosen a flat bar when the crowbar struck the employee's right eye. The employee was hospitalized and required surgery.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
