An employee was pushing a 45-pound sleeve off a cylinder on a printing machine after it had become stuck. The actuating arm activated and pushed the sleeve, crushing his finger and resulting in an amputation to his right little fingertip. The machine was not locked out at the time of the incident.

An employee was teaching other employees how to pull a machine's shaft manually. With the machine turned off, the employee manually released air from the shaft. The shaft was pulled from the other end, and a finger on the employee's right hand was caught between the shaft and the core. The finger was lacerated and partially amputated.
