An employee was making a delivery at a customer's site. He was taking a box out of the truck and stepping back when he stepped off the dock and fell, landing on his hip. The employee sustained a fractured hip.

On June 13, 2024, at approximately 11:25 a.m., an employee was on the back of the company vehicle. He was closing the swing gates when a cart that was loaded on the truck started to roll toward him causing him to jump off the back of the flatbed. The employee sustained four fractures to his right knee/tibia.

An employee was wrapping product with 5,000 pounds of wire reeling while the product was on a racking system approximately 10 feet above the ground. The wire reeling fell off the racks and onto the employee's right thumb, resulting in a partial amputation.

An employee was diagnosing the cause of a rear light malfunction on a forklift when the reverse gear engaged. The forklift moved backward and contacted the employee causing a metatarsal fracture in their left foot and injury to their lower right leg.

An employee was collecting shopping carts in the parking lot when he was struck by a customer's truck as it was backing out of a parking spot. The truck pinned the employee's left leg against the cart mule they were using. The employee was hospitalized with a fractured leg and required surgery.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee took a picture of the seal and license plate on a delivery truck after loading. The truck then reversed back to the dock and pinned the injured employee against the dock. The employee sustained an abdominal fracture.

An employee was between a milling machine and an 18-wheeler belly dump when the 18-wheeler backed up and ran over his feet and legs. The employee was caught between the front of the milling machine and the truck. The employee sustained fractures to their hip, leg, and feet.

An employee was cutting wire when his gloved left hand got pulled into the counter on the wire machine resulting in amputation of his left little finger and a laceration to his right ring finger.

An employee reached under a raised truck to pull on the spare tire when the jack slipped, causing their hand to be caught between the fallen truck and the spare tire. The employee was hospitalized with a severe injury to the right hand.

An employee was replacing a hydraulic cylinder on a mini-excavator. The employee was positioning the machine while it was in a trailer when their right leg became pinned between a track and the trailer, resulting in severe lacerations to the lower leg.

An employee was using an 8-foot folding ladder to retrieve a 250-foot coil of wrapped wire on a shelf. The employee dropped the wire which struck the ladder, causing the employee to fall from the ladder to the concrete floor. The employee sustained a skull fracture, a right knee fracture, and a rib fracture.

Four employees were moving a wooden spool of fiber-optic cable fiber when the injured employee slipped and fell, twisting his ankle. The employee suffered three fractures to his ankle.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.
