The employee had just removed the arm of a skid steer from a fixture using a chain hoist with a hook and placed the arm on the floor. After the arm was placed on the floor, the employee removed the hook and began raising the hook with the hoist. While raising, the hook caught on the side of the arm causing it to turn over onto the employee's lower left leg and fracture it.

An employee was walking around and performing basic job duties on assembly prior to shipment. They drank from a water bottle that contained windshield washer fluid. The employee sustained stomach injuries and was hospitalized.

An employee was using an overhead crane with a two-leg chain sling to lift a loader arm sub assembly. One of the two legs was not attached and was secured on a jib crane in the area when the load was lifted, causing the loader arm sub assembly to fall and strike the employee. They sustained right foot and leg fractures.

An employee was on a surface two feet from the floor, checking the quality of a skid steer. As she pulled on a pin in the skid steer, she slipped and fell onto the floor, resulting in injuries to the right shoulder, face, and head.

M1 Support Services

The injured employee was assisting in the disassembly phase after a radar array had been lowered and secured. The crew began removing load bearing pins from an overhead crane to free the radome. A load bearing pin was stuck. The injured employee went to remove the pin manually as a second team member applied pressure from the opposite side. The pin unexpectedly released and struck the injured employee s right thumb, resulting in partial amputation of the distal phalanx including an open distal phalanx fracture and nail bed laceration.

TECHO-BLOC CORP

An employee was shoveling excess material (spillage) that consisted of dry mix, sand, aggregate and cement. As he was shoveling, some material blew back in his face and around his safety glasses. He got a piece of metal in his left eye that required surgery. The employee was hospitalized.

Jadwin Lumber Company

An employee was feeding boards into an edger. They fed a board in and then adjusted the blade for a second board that was wider. When they fed the second board into the edger, it hit the first board and kicked back, striking the employee in the chest. They sustained injuries including a spine fracture, an injured pancreas, as well as potential injuries to their spleen, liver, and intestine. The employee was hospitalized and required surgery.

An employee was helping an operator to pull a cable off a winch. The operator pulled in the line when it hooked some tree brush. The brush then whipped around and struck the employee's arm, resulting in a broken arm.

A co-worker was adjusting an overhead bracket with an impact wrench when the tool flew out of their hand and struck the injured employee in the face. The injured employee sustained an injury to their left eye.

Riverside Industries LLC

An employee was working behind a stack of steel I-beams (stacked 2 columns deep and 4 levels high on the floor) while a forklift was pushing the stack. When the forklift took off the second-to-last beam, the last one was pushed over, causing it to fall to the ground and strike the employee. The employee sustained a fracture to the right leg.

Bobcat Company

An employee was lifting a flat sub (sheet steel) in the flats robot cell (weld area). He was operating a hoist when his left little fingertip got caught between the lifting device hook and the flat sub. The employee sustained a partial transphalangeal amputation.

Altec Service Center

An employee was underneath a 35-foot boom truck and adjusting the transmission linkage when the truck jumped over the wheel chocks and rolled over the employee's right leg, resulting in a fracture.

GRADALL INDUSTRIES, INC.

A crane was being used to lift a debris tank from a rack. The tank crushed the employee's right hand, causing amputations to the employee's right ring finger and little fingertip.

Manitou Equipment America, LLC

An employee was removing a small sheet metal part out of the die of the brake press. The foot pedal was pressed, causing the part to lift and crush their left thumb against the die. The employee suffered an amputation to the thumb tip including the loss of flesh and the fingernail.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
