An employee was troubleshooting a defect in a mold base. The employee was checking for jammed plastic material in the mold when the clamp frame dropped. Their right middle finger was caught between the clamp frame and the mold base. The employee sustained a fingertip amputation and open fracture of the tuft of the distal phalanx.

On April 27, 2025, an employee was going to assist with a jam where the plastic (PET) sheet was not moving to the grinder. The employee was walking over when the sheet caught their leg and caused them to fall on the concrete floor. The employee sustained a fractured hip.
