On June 18, 2024, an employee was standing on a lift while detaching a beam from a hook. As they reached up to grab the hook, they felt a strain in their abdomen. The employee was hospitalized with a hernia and required surgery.

On June 2, 2022, an employee was in a scissor lift, loading a piece of industrial equipment onto a flatbed truck. The employee was disconnecting a four-leg sling when one of its legs caught the bottom of the scissor lift, which was lifted about 8 inches. The employee fell from the scissor lift to the ground 10-12 feet below and suffered fractured vertebrae, a dislocated thumb, and a cracked rib. The employee was hospitalized.
An employee bent over to turn on a machine when she heard a pop in her lower back and experienced pain shooting up her back. When standing up, she could not put pressure on her left leg. The employee was hospitalized.
An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.
An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
