Final narrative
An employee was removing 4x4 stacked boards from bundles of pipe. A bundle of pipe fell, broke apart, and struck the employee's left leg. The employee's leg was fractured.
A crane was hoisting a pipe. An employee was adjusting twisted lifting chains on the end of the pipe when the pipe was raised, causing the employee's hands to be caught between the chains and the pipe. The employee suffered injuries to the left index finger, left ring finger, right ring finger, and right index finger; the right index fingertip was amputated without bone loss.
An employee was adjusting a steel strip in an end weld table. The shear to cut the strip was activated and amputated the employee's right middle and little fingers.
An employee was moving stacks of bundled 12" square structural steel pipe. There were three bundles atop each other. When he was cutting the last banding on the top bundle, a piece of steel fell and hit his legs and feet causing fractures that required surgery.
