The injured employee got out of his truck and began rotating the two-yard compactor by hand in order to position the can for service. While moving the compactor, the metal lid folded backward toward the employee's hands, crushing and fracturing his left index and middle fingers. He was hospitalized for surgery.

An employee picked up a plastic 10-gallon recycling bin and carried it over to empty it into the bunker. He walked back to his position holding the empty bin. While walking, he tossed the bin down to the floor from waist level. The empty bin bounced off the floor and tipped over, causing him to trip over the bin and fall on his left side. The employee sustained a fractured femur bone. The employee was hospitalized and required surgery.

An employee was conducting maintenance on a vehicle. He was replacing torque rods underneath the vehicle when the differential moved and lacerated his right hand resulting in hospitalization.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was performing maintenance on a hopper. The employee's hand was caught in a cylinder pivot point area when the pneumatic cylinder released trapped air. The cylinder crushed the employee's left little finger, causing the partial amputation of the fingertip.

An employee was opening a door to a semi trailer at the facility and his hand was caught between the door and another parked semi. The employee's left index finger was fractured and thumb muscles were injured. The employee was hospitalized and required surgery.

An employee was removing residual meat particles from the sprocket of the crossover conveyor belt in the packing area. While doing so, the conveyor started and his fingers were caught between the metal framework and the belt, resulting in amputations to the middle and ring fingers on the left hand as well as skin damage to the right hand.
